Health Card
Prior to your child's first day of school, you will be given a Health Information Sheet to complete. This is very important. Please print all information clearly. This informs the school staff of any health issues that your student may have as well as all of your contact information.

Contacting Parents
If your child becomes ill or injured at school. We MUST be able to contact you. PLEASE keep all contact information current. If at any time throughout the school year, your phone number or address changes, please contact the school for your information to be updated. We cannot send a child home with anyone until a parent has been notified. If your student has an emergency or needs immediate care that cannot be provided at school, those contacts that you list on your Health Information Sheet will be called, IF we cannot reach a parent first. Please understand that this is for your child's protection.

Services

Vision Screening
All students will be screened annually for vision problems. If your student does not pass this screening, you will be notified by letter that he/she needs further evaluation by a health care professional. Please have your provider complete this letter and return it to the school.
Hearing Screening
All students will have their hearing screened annually. If your student does not pass this screening, they will be re-screened at a later date. If he/she does not pass the second screening, you will be notified by letter that your child needs further screening by a health care professional. Please have your provider complete this letter and return it to the school.

Scoliosis Screening
All female students in grades 5-6 will be screened by Polk County Public Health for a curvature in their spine. You will be notified by letter of any concerns found by the Public Health Nurse.
Puberty Education
All 4-6 grade students will participating in puberty education led by Polk County Public Health. If you do not wish for your student to participate, please send a letter to the school excusing them from this.

Policies

Sick Policy
If your student appears to be or complains of being sick, the school nurse will take their temperature. If they have an elevated temp, a parent will be called to pick them up as soon as possible. Or if they vomit, a parent will be called to pick them up. They may return to school 24 hours after the fever has broken. If the child does not have a fever, they may lie down in the sick room for a short time to see if they start to feel better. Sometimes, all they need is a short rest and a drink of water to be able to feel better. If they state they don't feel better, a parent will be called to pick them up.

Lice Policy
If your student is found to have live lice, it is required that a parent picks them up as soon as possible. They may return to school the following day IF all the live lice have been removed and appropriate lice treatment is completed. Follow treatment directions EXACTLY and repeat following package instructions if necessary.

First Aid
Administration of first aid will be in accordance with school district procedures and guidelines, the need for further treatment beyond these recommendations will be referred to parents. The school health aide does not diagnose or prescribe treatment, but may suggest contacting the student's health care provider. Final decisions regarding the need for a provider's evaluation will be the responsibility of the parent.
